Indications/Uses
Major depressive episodes. Prevention of recurrence of major depressive episodes, panic disorder, w/ or w/o agoraphobia, obsessive compulsive disorder in adults & childn 6-17 yr, social anxiety disorder, PTSD.
Dosage/Direction for Use
Depression & obsessive compulsive disorder Initially 50 mg/day. Dose changes should be made in 50 mg intervals of at least 1 wk. Max: 200 mg/day. Panic disorder, PTSD, & social anxiety disorder Initially 25 mg/day. Dose changes should be made in 50 mg intervals of at least 1 wk. Max: 200 mg/day. Childn & adolescent w/ obsessive compulsive disorder 13-17 yr Initially 50 mg once daily, 6-12 yr Initially 25 mg once daily. Dose may be increased to 50 mg once daily after 1 wk. Max: 200 mg daily.
Administration
May be taken with or without food.
Contraindications
Hypersensitivity. Concomitant treatment w/ irreversible MAOIs due to risk of serotonin syndrome w/ symptoms eg, agitation, tremor & hyperthermia. Do not initiate for at least 14 days after discontinuation of treatment w/ irreversible MAOI. Discontinue for at least 7 days before starting treatment w/ irreversible MAOI. Concomitant intake of pimozide.
Special Precautions
Serotonin syndrome or NMS, switching from SSRIs, antidepressants or anti-obsessional drugs, other serotonergic drugs eg, tryptophan, fenfluramine & 5-HT agonists, QTc prolongation/torsade de pointes, activation of hypomania or mania, schizophrenia, seizures, suicide/suicidal thoughts/suicide attempts or clinical worsening, sexual dysfunction, abnormal bleeding/hemorrhage, hyponatremia, w/drawal symptoms seen on discontinuation of treatment, akathisia/psychomotor restlessness, diabetes, electroconvulsive therapy, grapefruit juice, interference w/ urine screening tests, angle-closure glaucoma. Hepatic & renal impairment. Not recommended in pregnancy & lactation. Childn. Elderly.
Adverse Reactions
Insomnia; dizziness, headache, somnolence; nausea, diarrhea, dry mouth; ejaculation failure; fatigue. URTI, pharyngitis, rhinitis; decreased appetite, increased appetite; anxiety, depression, agitation, decreased libido, nervousness, depersonalization, nightmare, bruxism; tremor, movement disorders (including extrapyramidal symptoms eg, hyperkinesia, hypertonia, dystonia, teeth grinding or gait abnormalities), paraesthesia, hypertonia, disturbance in attention, dysgeusia; visual disturbance; tinnitus; palpitations; hot flush; yawning; dyspepsia, constipation, abdominal pain, vomiting, flatulence; hyperhidrosis, rash; back pain, arthralgia, myalgia; irregular menstruation, erectile dysfunction; malaise, chest pain, asthenia, pyrexia; increased wt; injury.
Drug Interactions
Irreversible MAOIs (eg, selegiline), reversible, selective MAO-A inhibitor (moclobemide), reversible, non-selective MAOI (linezolid), pimozide, CNS depressants & alcohol (carbamazepine, haloperidol, or phenytoin), fentanyl, other serotonergic drugs (including other serotonergic antidepressants, amphetamines, triptans), & w/ other opiate drugs.
